Graduation Forms

The following forms can be obtained from Terri Brennen in EB 163, or they can be downloaded using either Microsoft Word or Adobe Acrobat Reader.

• Petition to Graduate Form 5-3-1 in Word or in Adobe PDF.
  Use if you have one support area.
  
• Petition to Graduate Form 5-2-2 in Word or in Adobe PDF.
  Use if you have two support areas.

The G.A.T.E.S. Graduation Audit & Transfer Evaluation Services office, down the hall from the registrar's office, receives the petitions:
Summer Semester : Graduation Petition Deadline April 10
Fall Semester: Graduation Petition Deadline July 1
Spring Semester: Graduation Petition Deadline November 1
